SNAP Credentialing helps healthcare providers build a verified credentialing passport and share it, with their explicit consent, with healthcare facilities. This policy explains what we collect, why, and the control you keep over it.
What we collect
- Identity & professional information you provide: name, date of birth, NPI number, license and certification details, education, work history, and malpractice history.
- Credential documents you or your sponsoring facility upload: licenses, DEA certificates, certifications, CVs, insurance face sheets, and CME certificates.
- Account information: email address and, if you use them, Sign in with Apple or Google identifiers. Passwords are stored only as salted hashes.
- Access records: an audit log of every upload of, and access to, your documents.
What we do NOT collect
- No patient data or medical records — your documents are professional credentials, not clinical information.
- No advertising identifiers, no cross-app tracking, no analytics brokers.
- No location data.
How your information is used
- To build and maintain your credentialing passport, including automated reading of documents you upload so details can be pre-filled for your verification.
- To share your passport with a healthcare facility only after you approve that facility's access. You can see and revoke every grant.
- To alert you and facilities you have approved before credentials expire.
- We never sell your information. We never share it with anyone you have not approved, except as required by law.
How it is protected
- Documents are encrypted at rest with customer-managed keys (AWS KMS) and in transit with TLS.
- Document links are short-lived and every access is written to an audit log.
- Facility access is scope-limited and consent-based; revoking a grant takes effect immediately.
Retention and deletion
Your data is retained while your account is active. You can permanently delete your account at any time from inside the app (Home → Delete Account); this erases your profile, credentials, documents, and stored files. Facilities that previously accessed your passport may retain copies they lawfully obtained while your grant was active, as required by their own credentialing record obligations.
Your choices
- Approve or deny every facility access request; revoke at any time.
- Export or view everything on your passport from the app.
- Delete your account and data from the app at any time.
Children
SNAP Credentialing is for licensed healthcare professionals and is not directed to anyone under 18.
